国民健康保険料納付証明書What is it?
Issued by your municipal office, it certifies that you have paid your National Health Insurance (NHI) premiums for a given period. Immigration authorities require it as proof that your health insurance contributions are settled in full and on time.
When is it needed?
- Permanent residency application (health insurance compliance check)
- Work visa renewal or status change
- Year-end tax adjustment or income tax return (NHI premiums are tax-deductible)
- Rental contract or public housing application
Where to Obtain
City Hall Counter
Visit the National Health Insurance Division (国民健康保険課) of the municipal office for your registered address. Bring your insurance card and ID; staff will look up your payment history and issue the certificate on the spot.
Requirements:
- Residence Card (Zairyu Card) or My Number Card
- National Health Insurance card (国民健康保険証)
- Issuance fee: Approx. ¥300 per copy (varies by municipality)
- If requesting on behalf of someone else, bring a letter of proxy (委任状)
